The per-flight cost is the variable cost of operating each flight - I guess you could say it's things like replacement parts, external labor, inspections, fluids like oil and such.
Family maintenance is the fixed cost of operating a particular family - depending on the size & type of aircraft there are certain requirements - you may need specialized tools or equipment, rentals, supplies and such. You could maybe even throw depreciation into there.
I don't necessarily have a list of what's included and what isn't, for gameplay balancing purposes it's just broken down into general fixed and variable costs. Maybe someone that is familiar with the industry could give a detailed list of what is included, I know we talked about it in the chat a few months ago but I forget the details of what was discussed.
