The definition of a fifth freedom flight is the ability to sell tickets on all legs, which would include the JNB-DUR route. I don't know what freedom this JNB-DUR flight is, but that is allowed - it's a much different feature though.
Refueling stops will eventually be added I think. Just get A340-500s for now
